Allstate Power Systems, L.L.C. will provide the customer, upon the execution of this contract, regularly scheduled preventive maintenance on the specified equipment below. Attached is the “Preventive Maintenance Procedures For Inspections and Service Visits” document, which lists standard peventive maintenance procedures to be preformed semi-annually; which includes (1) service visit and (1) inspection visit for an annual contract.

The cost for preventive maintenance service will be billed at time of service. These charges can subject to change at time of renewal. Tax will be added if applicable.

This contract covers the standard preventive maintenance prodecures which are summarized in the “Preventive Maintenance Procedures For Inspections and Service Visits” document. Any additional service over and above said contract will be billed to the customer on a customer approved estimate prior to Allstate Power Systems, L.L.C. commencing work.

Allstate Power Systems, L.L.C. will contact the customer at the beginning of scheduled month of preventive maintenance service. Specific details on each unit under contract are listed below.
