CDPHP HRA Administrative Services Agreement
The Mayor is authorized to sign the Ancillary Administrative Services Agreement with Lifetime Benefit Solutions, Inc. to administer CDPHP's Traditional Health Reimbursement Account (HRA), contingent on Lifetime Benefit Solutions, Inc. being the only HRA administrator company contracted to work with CDPHP.[21]

Employee Assistance Program (EAP) Contract
The Mayor is authorized to execute the annual renewal contract with the Employee Assistance Program (EAP). The EAP is required because the Village has Public Works staff with CDL licenses and is available to all employees.[14][18]
Health Care Opt-Out Payment — Individual Coverage
Non-union full-time employees who opt out of individual health care coverage shall receive an opt-out payment of $2,000, paid as a single check at the end of the insurance year (May–April).[15]

A dedicated checking account funded with $22,707.20 (CD check 23682023) was established to pay health insurance deductibles as they arise for Village employees.[2]

Health Insurance Deductible Limits
Health insurance deductibles shall not exceed $1,000.00 for highway employees and $750.00 for police and office employees. Employees must submit receipts to the Village Clerk for reimbursement.[1]
